Answer: Access LicensePa (the link above), enter either the license number or the name of the licensee, access the license record. Disciplinary actions, if any, are noted at the bottom of the screen.
Question: How do I file a complaint against a licensee?
Answer: Access the appropriate board Web page under Consumer Information to download the complaint form and directions for filing.
